Sliverline vinyl parts needed

In trying to unstick a stuck sliverline vinyl double pane double hung window I broke the adapter that holds the pins that is part of the side brackets (balances?) and the one of pins that go into the side balances.
Please see the pictures of the parts I need.
One of the plastic parts in the balance reads, "AmesburyTruth.com"
I also would need screws that go into the plastic holder that keeps the pins in place in the bottom of the window.

Hello there. This looks like a 15-195 T-Lock Shoe, used with Series 795 Channel Balances. The pivot bar looks like the 26-083, just review the dimensions to confirm it matches up. For fasteners, the 33-074 might work. Take a look and let me know if you have questions.
