Need the white slide part and the metal "T" or insert that is screwed into the bottom of the sash.

White slide looks to be about 1 1/4 " wide and about 1/2 " thick.

We have hardware that looks similar to yours but not exact replacements. I'm going to have one of our senior experts research your hardware and see what the best replacement options would be. Thank you!

I'm afraid I'm having trouble tracking this pivot shoe down. Since this connects to a spiral balance, you may be able to use our 15-129 as a substitute. This is 1-1/4" but 9/16" thick. I cannot guarantee that it will work, but I was unable to find anything closer.
