Please see our 79-097 pivot set, popularly used on US Aluminum commercial doors. This might be the pivot set used on your own door, but check to see if the mounting holes and other dimensions match up to your original first. 

Also be sure to take a look at the related video on commercial doors. Your door seems to be dragging on the bottom threshold, so you may want to try to adjust the bottom pivot to raise up the door to keep it from dragging.
