This cover appears to be the 15-205 Coil Triple Support. These are compatible with the 3/8" wide Series 165 Egress Coil Balances. It appears the coils have come disconnected from the balance shoe, so you may want to consider replacing the other hardware as well.

Take a look at the video tutorial below for instructions on how to remove/install the assemblies.
