Top Sash Guide Repair / Replacement

The top sash guide for my window (H-LC30-53-96) has broken on both sides letting the balance springs pop up making it difficult to open the window.  I need help finding the correct replacement parts (top sash guide and pop-out clip)

For the take out clip, see our 16-002. I'm not sure what top sash guide you need, though. Are both broken, or just one? If you have one that isn't damaged, please remove it from the sash and show us a few pictures of it. I will also need detailed measurements.
