Thanks for posting. This looks like either the 39-253LB or 39-438B operators. The difference between these two operators is the appendage used at the end of the straight arm. The video below goes into more detail. In either case, for a new cover and handle use the 39-375 or 39-451, depending on the style you prefer.

Hello there! This looks like the 39-220B and 39-221B Dyad Operators to me--review the dimensions and see if you think these match what you need. For new covers and handles, see the 39-375 and 39-375 or 39-451 and 39-452, depending on the style you prefer.
