It is stamped Truth Entry Guard, and has gears similar to 39-036, but the operator's arms and link arms look different.

Thanks for posting! The 39-036 would not be the correct replacement for this balance since it is a dual arm assembly.

Take a look instead at the 39-031 EntryGard Dual Arm Operator; the opposite handed version is the 39-032. Carefully review and compare the dimensions below to confirm that this is the correct replacement.
