Is the metal channel, not including the nylon fittings, 35" in length? If so, see either our S380-35 or S385-35. The only difference between these two balances is the bottom nylon shoe depth, or 'heel-to-toe' measurement, so compare both with your old hardware to see what matches best. The picture below illustrates the difference. For a stamp option, I believe you will need 3430 based on that stamp. This supports a window sash in the weight range of 18 through 23 lbs.
