Window Balance 28 3 IL Caldwell

The code on the balance is too light to get picture of. The stamp reads as follows: 28 3 IL Caldwell. Each balance is 2'5" without the attachments.

The reason for replacement is because the windows will not hold up. The continue to slide down when opened.

Please help me identify which parts I need for replacement.
