Window Channel Balance

A handy person from Indiana says:
Trying to determine to proper replacement part for a window channel balance. The part is white and is 34? long with a 33 REG stamped in it. The channel balance is screwed into the top of the vinyl window jam (so not slid in to the jam) and the string is attached to a balance knife lock shoe (white) at the bottom of the window.

I think we can replace this balance, but it will have to be a special order. Before we begin, can you give me the exact weight of the sash? Would you also be able to show me a photo of the shoe at the end of the cord? I suspect it's our 15-014, but I wanted to be 100% sure. Finally, how many did you need?
