The casement operator appears to be either our 39-253LB or 39-438B. The video below demonstrates the difference. Now, as far as the window hinge goes, I don't believe the 13" hinge is made anymore, but I think you can use the 36-004-14 14" Hinge Set to replace this. Just be sure to replace both hinges in the window at the same time.
