Greetings. I'm afraid we don't carry the two-piece top sash guide. To help find a suitable replacement, may we have the width, depth, and height of the black nylon piece, as well as the width and depth of the metal piece? This information could help us narrow down a replacement for you.
As for your balances, they look like our Series 390 Channel Balances. If the lengths of 17" and 30" are just for the metal channel, and not the end attachments, then you are looking for the S390-17 and the S390-30. You will need to the stamped numbers on the metal channels when ordering to select the correct stamp option. These numbers correspond to the weight range of the window sash that you need the balances to support.
