Hello! I'm the colleague Bob mentioned. The company that made your balance is no longer in business, but we can help. We have an S795-27 tilt balance with a 1-1/4" wide bottom shoe that should work as a substitute, however ours screws in at the top while yours appears to use a hook. 

What we can do is custom make this balance to use our 15-198 top hook instead of the screw tab it comes with. If you were interested in this, let me know how many you need and I can email you an order link and price quote for that quantity.

Oh, and before I forget, we'll also be using a 27B stamp option. This indicates a sash weight of 11 through 17 pounds. You may want to weigh your sash just to confirm that it falls in this range. Let me know!
