Thanks for reaching out. Your balance looks like a match for the Series 795. You'll have to measure the metal channel to determine the length and weigh the window sash on a scale to determine the proper stamp option. The 15-195 with a closed cam is the shoe our balances come with so you'll want to remove the old one to confirm it matches up to ours. 

Note that the S795 installs with a screw. If yours uses a hook, please let us know as this would have to be a custom order.
