Thanks for reaching out to us. Take a look at the S380-29, S385-29, and S378-29 channel balances. The difference between them is the depth of the bottom shoe so you'll have to measure as shown below. We recommend measuring an undamaged shoe for an accurate measurement. 

You may also need the 2820 stamp option based on your old one. You can weigh the window sash on a scale to confirm whether or not it falls within the 12 to 16-pound range.
