Thank you kindly for bearing with us during our busy season. Unfortunately, this sticker doesn't help us ID your hardware. Can you upload photos of the pivot shoe? We'd also like to see a photo of the pivot bar. 

Thank you! We don't have a direct match for this pivot shoe, but the 15-199 is similar. Take a look at the specifications and see how they compare with yours.

The compatible coil springs are the 1/2" S160 and 3/8" S165. Can you measure the width of the old spring band and tell us what you get?

Thanks for confirming! In that case, you'd want the Series 165; specifically, the S165-6, since your old springs are stamped with #6.
