Thanks for posting. This operator resembles either our 39-253RB or the 39-439B. These operators are identical save for the attachment at the end of the straight arm. The 39-253RB used a square track slide, the 39-529 sold separately; whereas the the 39-439B uses a rectangular slider that comes with the assembly. The video below illustrates this difference. In either case, for a new cover and operator you can use either the 39-376 or 39-452, depending on which finish you prefer.
