Thanks for posting. Start by looking at our 26-040 or 26-068. It's a bit tough to tell the precise hole spacing dimension from the photos, but compare and see if you think either of these could work for your sash. The 26-068 does not come with a fastener. It's tough to say what you'd need since the right screw can depend as much on the sash as it does the pivot bar. You'd need to take an old one, if not from this window then from an identical window in the house that has the screws still intact, and then go to a hardware store to find the correct replacement.
