I believe this is patriot window, not 100 0/0 sure please help identify this product to order I have 57 double hung windows to service thank you I was talking with Tim on the phone please reference this email to him

Based on your photos, I believe our Series 795 channel balance could work for you. Keep in mind this comes with a 1-1/4" wide shoe and screws to the window frame at the top. The pivot bar looks to be our 26-360.

Unfortunately, that keeper is nowhere to be found. I've seen it a few times in the past but my research indicates that the mold is lost. I couldn't find anyone who still has this. The closest keeper is our 90-257, though I cannot guarantee it will work for you.Please compare it with your parts and see what you think.
