Hello there. It looks to me you have either the 39-032 (its opposite-handed version is the 39-031) or the 39-156 (the 39-155 is the opposite-handed version). The only difference between these two sets of operators is the length of the link arm. See the illustrations below for reference. Compare both with your old hardware to see what matches best. In either case, for a new cover and handle see the 39-107 and 39-114L sold separately.
