Thanks for writing in! Your balance shoe is quite likely to be a match for 15-177 Coil Balance Shoe. To confirm, carefully look over their specifications and compare them. You'll need to remove the shoe.
The pivot bar reminds me of the 26-226.
This shoe uses the 1/2" wide Series 152 Coil Balance; attached using the 33-072 screw. If possible, you'll need to reuse the existing supports for the repair. If you cannot reuse them, you'll need the 15-143 Coil Bushing, 15-144 Coil Bushing Cap, and 15-145 Coil Support Barrel. All three snap together to create the support.
You'll need to check the existing coils for any stamps/markings; this will tell you the coil weight option needed. If yours do not have them, you'll need to confirm the exact weight of the sash (the window the balances are supporting).
