This top sash guide looks like our 18-073, which was used by Fiberlux Window Company. Please compare our dimensions to yours to confirm.

As for the balance, our Series 350 may be a good match, but you'll have to carefully compare our information to yours to be sure. I can't really tell from your photos if the Series 350 is the same.
