This looks like an S350-27 Channel Balance to me. Look it over to see if you concur. For a new stamp option, choose 2630. All costs are listed on the shop pages, and a selection of shipping options are listed when you put in your zip code at checkout. Let me know if you have further inquiries.

Thanks since I am not familiar with this. I will order the 2630 option. Is there anything else I need to order? I noticed there are some additional items such as channel balance shoe and top sash guide. I don't know about these things. Thanks

The shoe and top fitting come attached to the balance, so the balance is ready to install out-of-the-box. The Top Sash Guides are nylon pieces in the top corners of the window sash that hold the balances in place during window operation. If yours are undamaged, there's no need to replace them.
