The pivot shoe you have here is no longer made but the 15-177 Coil Balance Shoe will work fine to replace it. Be sure to remove the shoe and double check the dimensions to confirm.

The coil spring compatible with this shoe is the Series 152 Coil Balance, which is attached using the 33-072 screw. It would be a good idea to examine the coils to see if they might need to be replaced as well. 

There should be a stamp/marking on the coil that will indicate the strength of the springs. If not, you'll need to obtain the exact weight of your sash and divide that by the total number of coils in the window. You'll be able to use that number to select the spring from the list of S152 balances.
