To further review, can we see picture of the operator that these pivot slides attach to? We will need to see the base and arms. From there we can determine if we can provide a match. Thank you!

At first sight, your operator resembles the 39-453, but this style has front mount installation holes. Does yours have front or rear mount holes? Let me know! Compare the dimensions below to see how it compares.
The track slides are not sold separately.
