Do the weights (5 lb., 8 lb., etc) refer to sash weight, and can I determine by simply weighing my sash?

That's right. You would weigh your sash, then divide it by two (for each balance). For example, I see you were looking at our S140-20. This is for 40 pound sashes, since each side carries 20 pounds.

Keep in mind this is unique to coil balances. Other balances aren't rated this way.
