I need to replace a Broken one

In need of 2 sets of drawer slides. 12 inches long - larger pirce is 1" wide and the smaller piece is 5/8 - 11/16" & 1/32" high with both of them together. Please let me know. Thank you.

We had the plastic side spacers break on our Brookhaven roll out draw. I need to replace two. The item is 1.25 tall from the base. The bottom is two inches long (including the mounting holes) and the square part is .75".

These hinges are cracked and I need replacements. The number on the hinge is 674 305 5300...I prefer the exact model but if its not available, please let me know if you have something compatible that would work, I'd be fine with that as well. Any help is appreciated.

A plastic piece that holds the side track of my cabinet door broke. The plastic piece has the marking RSI.
