Your balance shoe reminds me of the 15-111 Pivot Shoe. To determine if this is the correct replacement, carefully review and compare the dimensions below. 

The compatible pivot bar is the 26-241.

These shoes are typically used with the 1/2" wide Series 160 Coil Balance. Since yours are marked with a 3 and a 6.5, I believe you'll need the S160-03 and S160-06. The support can be replaced with the 15-150D Coil Double Support.

Be sure to double check that the sash weight is around 19lbs.
