The pivot shoe reminds me of the 15-046 and if the balance tube is 5/8" thick, I recommend the Series 600 tilt balances. You will need the tube length and the color of the nylon end where the spiral comes out. We have related media guides on this page to further help you. 

Regarding the fogged insulated glass, the Patriot window manufacturer company ceased so we suggest calling a nearby glass company. They should be able to get you replacement insulated glass.
