I am checking which is the correct item that I should order based on the pictures above. I need two of them. Please review and let me know. Thank you.

Thanks for posting. I believe this is either an S351-27 or S390-27 Channel Balance. The only difference between these two balances is the depth of the bottom nylon shoe, or the measurement from heel to toe, so compare both with your old hardware to see what matches best. See the diagram below for reference. In either case, choose 2630 for a stamp option.

I measured the size, it's 1 1/8". I am going to order a set of total two pieces. Do I need to order related parts: 15-001? or there are already included. Please advise. Thank you.

Great! I'm glad to hear that the S390-27 is a match. There's no need to order the nylon fittings in addition to the balance, as they will come with the balance pre-attached. All of our channel balances come assembled and ready to install out of the box.
