Hi there. This looks like a Series 795 Channel Balance. We need a little more information to narrow this down further. You'll need to measure the length of the metal channel to determine the length you need. Show us a picture of any stamp code on the metal channel. Measure the width of the T-Lock Shoe at the bottom of the balance, and give us a clear picture of this shoe from the front. Finally, our standard Series 795 balances have a screw-on tab on top, but yours has a hook to attach to the window track. There's a few solutions to this; can you show us the hole in the window track that that hook attaches to? That will indicate which course of action we can take. Thanks.

Perfect, thank you! Looks like your balance hook goes into a square hole in the jamb. That's the best case scenario. That means we can avoid a custom order and you can purchase our Series 795 as is. You'll also need a 26-200 screw plug to pop into that square hole, allowing our Series 795 to screw into it.

The back of your shoe looks like it's solid to me, so you'd want the 15-195. This comes with the Series 795 already by default so there's no reason to purchase it separately.
