replacement for channel balance and pivot lock shoe

In the images, I included the failed pivot lock shoe part number 6.679.000,4.610.108,D467.490 I read that those parts are no longer made. I measured the failed part at 1" My channel balance has the markings 28C and is flat channeled. The balance channel also requires the "hook" as pictured. I did place an order for your 795 series, but the needed hook is not part of the standard, which I am returning. I will require the hooks (15-198) Can someone provide the custom order sheet?
