The window balances you have here are Caldwell balances but we have frequently seen them used in Milgard windows. Yours can be replaced with the S378-21, S380-21, or the S385-21. To determine which of these is the correct fit, you'll need to confirm the measurement for the depth of your shoe.

You can likely select the 2050 stamp option for the replacements but you may want to double check that the sash weighs between 23 to 27lbs.
