sliding glass door handle and double hook mortice lock assemble

Question

I need a sliding glass door handle and double hook mortice lock assembly. The double hook lock assembly broke on both ends as you can see in the attached photo, it's approximately 12 1/2 in long X 1 in wide and 1.700 in deep with DEC06 and PAT #6264252 and PAT #5.820.170 stamped on the sides. I would also like to replace the handle assembly which is white, 10 inch length X 1.290 in wide with only an internal latch for locking.
