Thank you for reaching out to us. We may be able to help with this. I'm just not entirely sure since your hardware is damaged. Would it be possible for us to see photos of an undamaged version of this balance removed from the window? Can you also confirm if the balance installs with a hook or a screw?

I believe this may be the 15-272 assembly but review the specifications and see what you think. We don't have a stock balance that uses this but we have a work-around. You can use the Series 795 as long as your old balance installs at the top of the window track with a screw. You'll just have to swap the default T-lock shoe with the 15-272 which is sold separately. 

You will also need to measure the metal channel to determine the length of your replacements, as well as weigh the window sash on a scale to determine the proper stamp option. Let me know if you have any questions!

It looks like the 15-272 is the correct assembly piece. Just to be clear, the balance (what I have been calling the rail) seems to work ok. Is it possible to just buy two new 15-272 pieces and install them in the old balance?

If not, our old balance/rail says "308 KF5 OMC 45" (see pic) and is 30" long if you include the .5" tab for the top screw. Are you able to provide a recommendation for a new balance to go with the assembly piece?

Yes, you can certainly just replace the 15-272 carrier assembly if it's the only thing that's broken.
