Thanks for reaching out to us. Our 90-180 sash lock comes to mind. This measures 2-1/4" on installation hole centers with a 3/8" backset.

The balance appears to match our Series 795. The default shoe is the 15-195 with a closed cam so you'll want to make sure your old one matches this. There should also be a stamp on the metal channel. Note that if your old balance uses a hook to install, this must be a custom order.
