Thanks for posting!  This part appears to be a T-Lock balance shoe typically used for a Series 795 Channel Balance.

Your shoe looks like it could be either the 15-195 closed cam shoe or the 15-195B open cam shoe. To confirm which shoe you have, you'll need to examine the metal cam from your shoe. You'll also want to be sure to confirm the dimensions below. They are the same for both parts, the only difference in the cam. Let me know if the width measurement is different than 1-1/4".

If you are looking to replace the entire balance assembly, I'll need to know the stamp from the metal channel as well.
