Thanks for reaching out to us. This appears to be a T-lock shoe. The 15-195 or 15-195B may be what you need depending on if your cam is open or closed. You'll want to use an undamaged version as a reference for the best match.

The 15-195 comes with the Series 795 channel balance by default. Please note that this installs with a screw. If your balance uses a hook for installation, this must be a custom order.
