Thanks for writing in. This looks like a Series 795 Channel Balance. We need a little more data to confirm exactly what you need, however. Confirm that the metal channel, not including the T-Lock Shoe on the bottom or the hook on top, is 27" long. You'll need to obtain the exact weight of your window sash to determine what stamp option to select. 

It appears your balance has a hook like our 15-198 attached to the top that it uses to connect to the window track. Our standard Series 795 balances use a screw-on tab instead. This may mean we need to custom build balances for you that have this hook; there may be an alternative solution however. May we see the hole in the window track that this hook attaches to? The shape of that hole will tell us what you need. 

Finally, the bottom T-Lock Shoe looks like our 15-195B Open Cam 1-1/4" Shoe. Compare to confirm. Once you get back to us with this information, we can get all this started for you. Many thanks.
