Please compare the nylon balance ends on your balance to the ones on our Series 395 side load balance. If they match, order S395-24 with a spring weight option of 2330 (the number from the metal bar) and S395-36 with an option of 3520. 2330 is the correct option (in case you want to check) for window sashes weighing 15 to 18 pounds. 3520 is correct for 13 to 18 pound sashes.
