This looks like a "T" shaped pivot shoe for a channel balance. Unfortunately, we don't have an exact match. It looks like our 1" wide 15-194 at first glance, but yours has a deep groove in the stem that would prevent ours from working. The 15-194 simply wouldn't fit in your metal channel.

This means that your only option would be to replace the entire balance. We sell the Series 795, which I'm confident will work for you, but there are two things to keep in mind. The first is that the Series 795 has a 1-1/4" shoe by default, which means you'll need to purchase the 1" wide 15-194 separately.

The second is that our Series 795 has a screw tab at the top. If yours has a hook, you'll have to let me know. 

We can supply you with a Series 795 that comes with a 15-198 hook at the top, but it will have to be a custom order. Is the 15-198 a match for the original? Can you also tell me the length of the metal channel, the stamp printed on it, and how many you wanted to buy altogether?

Can you also show me a photo of the hole where the hook installs? There may actually be a way around having to order a custom assembly, which is non-refundable, but it will depend on the shape of that hole.
