This looks like our Series 795 channel balance, but with a special 15-198 hook attached to the top. We can custom make this for you, but first look over this hardware to confirm that it would work. I'm thinking using our S795-32 with a 32C option. If you want to go this route, just let me know how many you need and I can send you a custom balance form and price quote.
