We'd also like you to confirm the stamp printed on the back of the metal channel. Additionally, how many balances are you looking to replace?
Thank you! Your balance is a match for the Series 795, but you'll notice that this installs with a screw. Yours uses a hook instead, which means this must be a custom order. We'll need to know the stamp printed on the metal channel, along with the number of balances you're looking to replace.
The shoe appears to be the 15-195 or 15-194 shoe, depending on the width, but you'll have to remove the old one to determine the best match. You first need to tilt the shoe in towards you to about 90 degrees, then twist it so that the ears can slip past the track opening.
The shoe balance width is 15/16.

It looks like D167-490 on the metal channel.
Hmm, that's a very strange stamp for this balance. Could you weigh the window sash on a scale and tell us the precise weight? That will allow us to confirm the proper stamp option for the replacement balances.

We have two stamp options for you to select from: 30B and 30C. A pair of balances with the 30B stamp will support a sash weighing 13 to 19 pounds, and a pair of balances with the 30C stamp will support a sash weighing 19 to 24 pounds.
