Balancers and latch for single hung Millard window

Question

I have 2 windows that the left side balancer when looking from inside home to the outside of home is broke. Along with 1 latch that was damaged due to the balancer failing. Would need 4 new balancers 2 for each window. Along with 1 latch for the broken broken latch on window in picture

We do not carry your exact balance with the curved metal channel, but the Series 795 can replace it. Do you have an undamaged T-lock shoe at the bottom of the balance that we could view? We need to know the width of this shoe and if it has a closed or open(see through) metal cam.

Does the balance install at the top with a hook? If so, we will need to custom assemble the 15-198 hook on the balance for you. Please confirm.
