Your balance resembles the Series 795 channel balance, but with the 15-194 T-lock shoe instead of the 1-1/4" shoe it comes equipped with. You will need to purchase the 15-194 shoe separately and switch it out before installation as shown in the video below.

Please note that the Series 795 channel balances uses a screw clip at the top for install. I cannot confirm how your original installs. I recommend checking an undamaged version to confirm. 

If your balance uses a hook instead like the 15-198, this is a custom configuration that we will need to custom make for you. Please let me know if yours uses a hook instead.
