A home owner from Eatonton Ga. says:
This is a tilt window it has a hook at the top that top which inserts in a slot. The bottom has a metal bushing which fits inside a plastic housing with a white plastic guide. The channel has a 27 B stamped on it.

Thanks for posting. This looks like a S795-27 with a stamp option of 27B, with one key difference: Your balance has that hook atop it (our 15-198), while our standard Series 795 balances have a screw-on clip instead. We can certainly custom make these balances for you; but there may be another solution. Can we see the hole in the window track that those hooks attach to? That will clue us in to our best course of action. Thanks.

Great. In this case, you can just use our 26-200 Screw Plug in that hole, to install the standard S795-27 with a stamp option of 27B. A last point is just confirm your old shoe matches the one that comes by default with this balance, our 15-195. Let me know if you have any other questions.
