It looks like you need both a left-hand and a right-hand commercial door pivot, right? Take a look at the 79-127 and 79-128. See if these match up to the pivots on your doors.
Take note: on the bottom pivot, you will need to use a lubricating oil, and you might also have to drill out the screwsheads in the threshold. Also, in some cases, you can leave that bottom threshold pivot pin in place and just replace the other parts on the doors. You may be able to do that, too.
