Your coil balance pivot shoe can be replaced with our 15-199.  I am confident these are the replacement shoes you'll need but be sure to double check and compare the dimensions.

Since your coil balance cover appears to be the 15-204 3/8" Coil Dual Support, you'll need the Series 165 Coil Balance to complete your assembly. The width of your coils should be 3/8" as well. 

To select the coils, you'll need to look for any stamps/markings that will indicate the spring strength needed for your window.

If there are no stamps/markings, you'll need to obtain the exact weight of the sash and divide it by the total number of coils in the window. That number will be the spring strength you'll need.
