Thank you for the additional photos. This is a match for the Series 795 but it must be a custom order since your balance uses a hook like the 15-198. I can't make out the stamp in your first photo, however. It looks like it may say 27B. Can you confirm that for us? Once I know that, I can send over a purchase link.

The 27" Series 795 channel will have the 15-198 hook attached with a 27B stamp that can support a 11-17 pound window sash. If there is any hesitance, I recommend weighing the window sash to confirm it falls in this range.
