Double coil cover?

Attached is photo of the window pn tags and a good coil cover. Not realizing, wife took screw out of one and it recoiled down and broke the cover on the other side into 8 pieces.

Can you help ID?

If the coils are 1/2" thick, then see either our 15-150D or 15-150T. If they're 3/8" thick, see the 15-150D or 15-150T. If you need to replace the entire system, it would be best to see it out of the window, along with the pivot shoe especially, to ID.
